Half damage of spinal cord can cause Brown Sequard Syndrome, resulting in paralysis and loss of proprioception on the same side as the injury or lesion, and loss of pain and temperature sensation on the opposite side of lesion.

Symptoms of Brown Sequard Syndrome

  • Loss of motor function
  • Loss of vibration sense and fine touch
  • Loss of proprioception
  • Loss of two-point discrimination
  • Signs of weakness on ipsilateral side of spinal injury

This is result of lesion affecting the dorsal column medial lemniscus tract which carries fine touch fibres, conscious proprioception and corticospinal tract which carries motor fibres. On the other hand, the loss of pain and temperature is resulted via spinothalamic tract.

Causes of Brown Sequard Syndrome

  • Spinal cord tumor
  • Trauma
  • Ischaemia
  • Infectious or inflammatory diseases such as tuberculosis or multiple sclerosis

It is not always permanent and progression or resolution depends on severity of original spinal cord.

Treatment for Brown Sequard Syndrome

Pharmacologic therapy: Indicated for spasticity pain or number of other possible complication.

Nasogastric tube insertion: Subsequent low wall suction may help to prevent aspiration.

Physiotherapy

  • Maintaining strength in neurologically intact muscles
  • Maintaining range of motion in joints
  • Preventing skin break down by proper positioning and weight shifting
  • Improving respiratory function by positioning and breathing exercise
  • Providing emotional and education support
